The Alaska Power of Attorney Form lets a person designate someone else to handle their financial matters, medical decisions, or any other tasks they would like to specify. The type of form you use depends on the objective that you would like to accomplish. Apart from the kind of powers that will be conveyed, each type of POA requires the same essential components. Information such as the name and address of the appointee, restrictions to the granted controls, and all necessary endorsements are documented within the agreement. Some forms will also require a notary public to confirm the document’s legitimacy.

Durable (Financial) Power of Attorney – For financial use only, this document allows someone else to make decisions on behalf of the principal. The extended permissions serve to remain in effect even if they become incapacitated.

General (Financial) Power of Attorney – Allows someone to convey all the same financial powers as a durable POA, but it does not remain active once the principal is no longer sound of mind.
