The Alaska Power of Attorney Form lets a person designate someone else to handle their financial matters, medical decisions, or any other tasks they would like to specify. The type of form you use depends on the objective that you would like to accomplish. Apart from the kind of powers that will be conveyed, each type of POA requires the same essential components. Information such as the name and address of the appointee, restrictions to the granted controls, and all necessary endorsements are documented within the agreement. Some forms will also require a notary public to confirm the document’s legitimacy.

Advance Directive (Medical POA + Living Will) – This form is becoming the standard for those interested in planning out what is to be done medically should they ever be in an unresponsive state.

Signing Requirements: The endorsement of the document must occur in the presence of a notary public or two (2) witnessing parties (§ 13.52.010).

 


Durable (Financial) Power of Attorney – For financial use only, this document allows someone else to make decisions on behalf of the principal. The extended permissions serve to remain in effect even if they become incapacitated.

Signing Requirements: The principal must perform their signature before a notary public (§ 13.26.600).

 


General (Financial) Power of Attorney – Allows someone to convey all the same financial powers as a durable POA, but it does not remain active once the principal is no longer sound of mind.

Limited (Special) Power of Attorney – A fitting document for anyone wishing to assign a specific power to another individual.

Minor (Child) Power of Attorney – If a parent or guardian needs someone to handle certain matters regarding their child, they can put forth this document to delegate a specified power that can be used to accomplish a task for the minor.

Motor Vehicle Power of Attorney – Used to elect someone else to handle a specific motor vehicle’s titling, registering, or transferring.

Real Estate Power of Attorney – Any individual who owns or would like to own a particular property may employ this form to have a trusted associate carry out actions in their name regarding a designated piece of real estate.
