Advance Directive (Medical POA + Living Will) – This form is becoming the standard for those interested in planning out what is to be done medically should they ever be in an unresponsive state.

Signing Requirements: The endorsement of the document must occur in the presence of a notary public or two (2) witnessing parties (§ 13.52.010).

 


Durable (Financial) Power of Attorney – For financial use only, this document allows someone else to make decisions on behalf of the principal. The extended permissions serve to remain in effect even if they become incapacitated.

Signing Requirements: The principal must perform their signature before a notary public (§ 13.26.600).

 


General (Financial) Power of Attorney – Allows someone to convey all the same financial powers as a durable POA, but it does not remain active once the principal is no longer sound of mind.
