The West Virginia Revocation of Power of Attorney serves to rescind the authority issued by a medical or financial power of attorney contract. Although many power of attorney agreements stipulate a termination date on the permissions granted to a third party, the revocation paperwork is also available to the principal in situations requiring early abolishment of the arrangement. The one-page form can be applied to various circumstances, even if the initial accord was considered “durable”. To file the revocation, the declarant of the primary power of attorney will need to indicate the classification of the original. Further preparation of the registry will prompt the principal to establish the previous filing details, specifying the nominated agent and the original date of execution. The principal’s signature can then be recorded with the assistance of a licensed notary to meet the state-mandated requirements to trigger its acceptance.
