West Virginia Advance Directive Form (Medical POA & Living Will)

329 Downloads

The West Virginia Advance Directive validates a written approval designating a person indicated rights for decision-making concerning another’s health care. It is standard practice to assemble this type of legal agreement to confirm an arrangement to highlight end-of-life desired care in situations that could leave an individual in an incapacitated state. The principal granting the controls will effectively transfer responsibilities to a trusted party in congruence with their own stated wishes within the filed paperwork. Qualifying the understanding contained in the contract will assign a mapped-out description of what the declarant would prefer if they are in a coma or vegetative state. Outside of the established selections in the document, the determined representative nominated in the form would then decide the next steps to take.
