The Nevada Living Will presents a structured account transcribing one’s choices concerning health care treatment options. The form allows a principal party to register their preferences on various medical scenarios to create a record for physicians should the patient become incapacitated. Indication of desires regarding life-support and pain management can be addressed within the arrangement, as well as sections to cover specific wishes for end-of-life care.

Statute – Nevada Revised Statutes – Registry of Directives for Health Care (§ 449A.600 – 449A.645)

Definition – “Declaration” means a writing executed in accordance with the requirements of § 449A.433 (§ 162A.730).

Signing Requirements – Completing the paperwork will call for two (2) sufficient witnesses to view the signature process (§ 449A.433-1).

Registering a Living Will in NV

All living will documents must then be secured with the state’s Nevada Lockbox, an online system designed to accept and store the legal commitment.

Step 1 – Complete the Living Will Form

The declaring party can fill the legal agreement as they see fit, initialing and signing where necessary. The document will then need to be signed before two (2) acceptable witnesses, and they must also provide signatures to substantiate the event.

Step 2 – Complete the Advance Registration Agreement Form

Fill the Advance Directive Registration Agreement form with all requested information. Endorsements from the principal and the person who prepared the document (if applicable) must be applied where indicated.

Step 3 – Submit All Forms to the Secretary of State

Once the living will and registration agreement are finalized, they can be submitted in one of the following manners:

After sending the forms, they will be received and reviewed for approval. Acceptance from the state will effectively instate and register the legal understanding.
