Michigan Living Will Form

The Michigan Living Will essentially is a statement of one’s preferences concerning medical care at the time of their incapacity. This means that the executor can establish what they want to occur regarding resuscitation efforts, life-sustaining treatments (artificial feeding, hydration, and breathing), medication, etc. It should be noted that this is not a legally binding document, but rather a list of instructions that family members, advocates, and doctors can refer to when making difficult decisions for the principal party. The form merely requires that the declarant insert their name, choose their preferences, and supply their signature (it is recommended that the primary individual receive the acknowledgment of a witness to further validate the form).
