The Michigan Advance Directive records an arrangement where somebody designates a “patient advocate” to make medical determinations for them in the event of their incapacitation. In addition to this feature, the principal may also declare how they wish certain health care scenarios to be managed when they are incapable of voicing their own desires. Users can fulfill the requirements of this instrument by reading through each page of the document, entering the requested details, and providing signatures within the necessary sections. Following the form’s completion, the principal should then furnish copies to their agent, physician, and any other relevant individuals.

Statutes – Designation of Patient Advocate (§ 700.5506 – § 700.5520), Living Will (No Legal Statute)
Definitions – “Patient advocate” means an individual presently authorized to make medical treatment decisions on behalf of a patient under sections 5506 to 5515 of the estates and protected individuals code (§ 333.5674(3)).
Signing Requirements – The section of the advance directive designating a patient advocate requires the principal to sign in the presence of two (2) qualified witnesses (§ 700.5506(4)).
