The Arizona Motor Vehicle Power of Attorney (Form 48-1001) is the official POA document provided by the Arizona Department of Transportation. This form enables an individual to supply another party with the right to obtain and sign paperwork regarding an automobile. This means that the beneficiary can act as the owner (or soon-to-be owner) and accomplish the registering, titling, purchasing, or selling of the primary party’s motor vehicle. The individual granting the powers must transcribe the following to the document:
- Vehicle Description (VIN, year, make, & body style)
- Personal Information of the Attorney-in-Fact (name & address)
- Personal Information of the Principal (name, driver license number, & address)
After supplying the instrument with the demanded details, the principal can conclude the arrangement by signing their name in the presence of a notary public or MVD agent who can then verify the form.
