Arizona Motor Vehicle (DMV) Bill of Sale Form

The Arizona DMV bill of sale form is a legal document that provides proof of purchase. Although not required by law in the state of Arizona, it can still be a very useful document in terms of protection. The form requires such information as the VIN number, selling price/date, seller’s signature, along with a notarized section. Once the form has been completed and notarized, a copy may be provided to both parties.

How to Register a Vehicle

You are not allowed to drive your recently purchased vehicle until it has been registered. If you need to drive the vehicle prior to registration, you can purchase a 3-day permit for a small fee by visiting the ServiceArizona website.

Buyer must appear in person to an MVD(Motor Vehicle Division) branch with the following paperwork:

  • A valid photo I.D.
  • Proof of ownership(Title) – If the title has been lost or stolen, complete the Arizona Title and Registration Form.
  • Proof of insurance
  • If the buyer lives in Phoenix or Tucson, they will need to provide an emissions compliance form that is issued during an inspection. (This is only required if the vehicle is older than 5 years.) – subject to § 49-542.03.
  • Be prepared to pay the required fees. (Registration fee: $8, Title fee: $4.)

How to Write

Step 2 – Vehicle Information – Provide the following vehicle information:

  • Vehicle Identification Number
  • Year
  • Make
  • Body Style
  • Sale Date
  • Sale Payment Amount

Step 3 – Buyer’s Information – Enter the information that is requested below:

  • Buyer Name (first, middle, last, suffix)
  • Driver License Number
  • Date of Birth
  • Mailing Address
  • City
  • State
  • Zip

Step 4 – Seller’s Information – Input the following required information. If the seller agrees to transfer ownership of the vehicle to the buyer for the sale’s price listed above, the seller shall then sign in the space provided.

  • Seller’s Name (first, middle, last, suffix)
  • Driver License Number
  • Date of Birth
  • Mailing Address
  • City
  • State
  • Zip
  • Signature

Step 5 – Notary Section – Once all the above information has been provided and the form has been signed, you must then bring the form to a Notary or MVD Agent to be reviewed and signed. This section requires the following information:

  • Notary or MVD Agent Signature
  • Date
  • County
  • State
  • Commision Expires
