The Wisconsin Minor Child Power of Attorney proposes to temporarily entrust the parental powers associated with a child to a surrogate person. Alternatively, the bestowed rights can be obtained upon death or incapacitation of a parent should they wish to amend the paperwork accordingly. The controls to sign and take action on the child/children’s behalf can also be useful in scenarios where the parent cannot be present. The document allows an individual to effectively enroll a child in school or manage medical decisions in the absence of the legal parent or parents. Additional state forms are made available below to initiate a petition for appointment guardianship within the court system. The principal must utilize an authorized notary official’s services to view and substantiate the legal contract’s signing.
