The Tennessee Revocation of Power of Attorney is the standard document used in cases where a granting declarant of an active power of attorney agreement wants to nullify the arrangement. In situations where service from a third party to act or make decisions on behalf of another is no longer needed, the contract can be rescinded using the supplied form. To revoke, it is necessary to complete the paperwork as requested, filling in the original issuance, agent, and principal information. The registry can then be signed as directed with proper acknowledgment from either a notary public or two (2) witnesses to certify the endorsement.

Signing Requirements – When the principal and attorney-in-fact apply their signatures, it must be performed with the help of a notary or two (2) witnessing individuals. (§ 34-6-203).
