The New York Revocation of Power of Attorney is an earmarked document presented to remove the issuance of controls granted by a previously granted power of attorney. For those instances where a principal does not further require the agency of a commissioned attorney-in-fact, the revocation POA allows for the legal repeal of the controls. Information on the declarant and the title of the original POA must be indicated within the appropriate blank spaces of the form. Finalization can be carried out once all requisitioned signatures are received, and submission of the agreement is sent to the former designated agent.

Signing Requirements – The original agreement’s principal party must sign the form to apply direct cancellation. It is advised to have two witnesses made available to confirm the event, as well as a licensed notary. The paperwork can then be delivered to the named agent to inform of the dissolution of authorization(s).
