The New Jersey Tax Power of Attorney is a state document that issues the defined representation from an individual taxpayer or business entity to an accountant or attorney regarding a specified tax account. The form is required for submission only when notification of request from the New Jersey Division of Taxation is received or if a tax professional needs to act on behalf of the taxpayer/company regarding state taxes. Basic returns/reports and fiduciary performance generally do not warrant the registration of a representative.

Signing Requirements – All associated taxpayers and assigned representatives are required to endorse the form.

How to File

The filing can be submitted in one of two ways, either by visiting the nearest NJ Division of Taxation or Regional Information Center or by mail. It is advised to make an appointment ahead of time on the NJ Division of Taxation website if submitting in person. All correspondence paperwork should be sent to the following location:
