The New Jersey Living Will provides a conventional prearranged confirmation regarding an individual’s preferences for medical treatment. The summary of wishes is prepared as a safeguard to establish a course of action should the principal party become incapacitated and no longer able to communicate their desires. Once the declarant has made selections, the paperwork can be signed and filed with a primary physician for future reference. It is recommended to make additional copies of the contract to be retained by the principal, an attorney, and/or a trusted family member.
