The Missouri Motor Vehicle Power of Attorney (Form 4054) was designed by the Department of Revenue to assist motorists with the process of delegating vehicle transactions to another party. In other words, if you’re an individual who needs somebody else to sell, title, or register a vehicle on your behalf, completing this form will you give you the ability to do so. To proceed, individuals must furnish the document with the basic details (information concerning the owner, agent, and vehicle) and select which powers will be granted. After the form is filled out accordingly, the owner(s) must sign the instrument and receive the acknowledgment of a notary public.

Definition – “Power of attorney”, a written power of attorney, either durable or not durable (§ 404.703(8)).
Signing Requirements – The principal’s signature must be witnessed by a notary public for verification. An electronic signature by the owner is only allowed if the power of attorney is being assigned to an insurance company due to total loss, in which case, the acknowledgment of a notary public is not demanded. (§ 301.644(1))
