The Indiana Revocation of Power of Attorney is carried out when someone wants to terminate the authority granted to another, whether the assignment has been accomplished or they just no longer desire an active agency. This is executed by supplying the form with information concerning the principal & agent, identifying the original POA, and signing/dating within the designated portions of the document. Although not required, this instrument provides areas within the form that allow for the acknowledgment of two (2) witnesses and a notary public to further validate the termination (signature of the principal is required).
