The Illinois Revocation of Power of Attorney effectively shuts down the arranged terms of a formerly executed POA document. This can be implemented at any time during the course of an active principal-agent relationship. To do so, the terminating party should deliver the form with the needed information, sign it in the presence of an official who has the ability to notarize the instrument, and then send copies to each individual who has an interest in the original POA document.
