The Illinois Minor Child Power of Attorney exclusively authorizes an individual to make healthcare decisions on behalf of the parents. If a mother/father/other legal guardian needs assistance caring for their child, they may capitalize on this form to ensure the additional caretaker has the ability to provide the minor with the adequate treatment they necessitate. Fulfill the requirements of the form by adding the information of the parties involved (parent(s)/guardians(s), minor child, & agent), inputting the details of the arrangement, and certifying the document with the proper actions (initialing, dating, & signing).
If you wish to supply someone with more wide-ranging parental powers that allow the individual to manage all aspects of the child’s life, such as school, travel, and extracurricular activities, you may want to consider carrying out the following as an alternative:
- Appointment of Short-Term Guardian (does not require the approval of a court)
- Standby Guardian Designation (requires the approval of a court)

Statute – General Principles: “The health care powers that may be delegated to an agent include, without limitation, all powers an individual may have to be informed about and to consent to or refuse or withdraw any type of health care for the individual and all powers a parent may have to control or consent to health care for a minor child.” (755 ILCS 45/4-3)
Definition – “Patient” means the principal or, if the agency governs health care for a minor child of the principal, then the child (755 ILCS 45/4-4(e)).
Signing Requirements – Acknowledgment of at least one (1) witness.
