The Iowa Revocation of Power of Attorney dismisses the conveyance of authority provided within the content of an active POA document. To initiate such a procedure, the revoking party will have to enter the following within the form; the names of all participating parties (principal, agent, & successor agent), a description of the original document (type, title, & date of effectuation), and their signature confirming the cancellation (acknowledgment of a notary public suggested). Upon fulfilling these required actions, the executor should deliver copies to their attorney-in-fact as well as any other parties or institutions who may have referred to the initial POA.
