The Iowa Medical Power of Attorney, or as the state labels it, the “Durable Power of Attorney for Health Care”, generates an official record of the assignment of a proxy to handle any affairs related to the executor’s healthcare. This will usually only take effect once the principal has been deemed incapacitated by an attending physician unless otherwise specified within the instrument. To implement this document accordingly, individuals must identify themselves as well as their attorney-in-fact, insert any supplementary provisions that they feel are necessary for the conveyance, and provide signatures while under the observation of two (2) witnessing parties or a notarial officer.

Definition – “Durable power of attorney for health care” means a document authorizing an attorney-in-fact to make health care decisions for the principal if the principal is unable, in the judgment of the attending physician, to make health care decisions (§ 144B.1).
