The Colorado Revocation of Power of Attorney is the written termination executed when a principal would no longer like their agent to retain certain previously granted authorities. To create a record of the cancellation, the terminating party will have to supply the form with the following details regarding the POA in question:
- Type
- Name/Title
- Effective Date
- Names of the Individuals Registered Within the Document (principal, agent, successor agent)
Subsequent to providing these details, the executor should supply the date of the revocation and sign it within the designated area.
