The California Minor (Child) Power of Attorney is a legal form that conveys certain parental powers to an agent who will be caring for an individual under the age of eighteen (18) years for a temporary period of time. The main objective of this document is to allow the attorney-in-fact to be able to perform certain acts concerning the child’s health care and education. To initiate the contract, the parent(s) or guardian(s) will have to record the name of the individual that they trust to act on their behalf, determine the various conditions related to the conveyance, and then provide their signature(s) within the indicated area of the form.

Signing Requirements – It is necessary that the principal sign the form in the presence of a notary public or two (2) suitable witnesses (AB-1163-4566(c)).
