The Arizona Living Will requires users of the form to be at least eighteen (18) years of age or older. The goal of instating this document is for the executing party to establish their medical choices if they are ever unable to communicate their desires. Elements that can be personalized to the individual’s preference include the protocol for the use of feeding tubes, life-sustaining treatments, and any other procedures that occur when a person becomes incapable of functioning on their own. Once you have customized the form to your wants and needs, it is imperative that it is signed before either a witness or notary public in order to make it official.

Statutes – Arizona Revised Statutes – Title 36, Chapter 32 – Living Wills and Health Care Directives (§ 36.3201 – 36.3296)

Definitions – “Living will” means a statement written either by a person who has not written a health care power of attorney or by the principal as an attachment to a health care power of attorney and intended to guide or control the health care treatment decisions that can be made on that person’s behalf (§ 36.3201(10)).

Signing Requirements – Signatures are to be performed in the presence of a notary public or witnessing party (§ 36-3221(3)).

Revocation – § 36.3202
