Arizona Living Will Form

An Arizona Living Will requires users of the form to be at least eighteen (18) years or older. The goal of instating this document is for the executing party to establish their medical choices if they cannot communicate their desires. Elements that can be personalized to the individual’s preference include the protocol for using feeding tubes and other life-sustaining treatments.

Statutes – Arizona Revised Statutes – Title 36, Chapter 32 – Living Wills and Health Care Directives (§ 36.3201 – 36.3296)

Definitions – “Living will” means a statement written either by a person who has not written a health care power of attorney or by the principal as an attachment to a health care power of attorney and intended to guide or control the health care treatment decisions that can be made on that person’s behalf (§ 36.3201(10)).

Signing Requirements – Signatures are to be performed in the presence of a notary public or witnessing party (§ 36-3221(3)).
