Step 2 -Vehicle Information – Fill out all data on the vehicle as follows:
- Vehicle Identification Number
- Odometer Reading
- Seller Registration Number
Step 3 – Denote in the proper box if you have attempted to file an application at another point in time for this vehicle.
Step 4 – Seller/Buyer Information – Enter all correct information about the persons participating in the exchange of the vehicle:
- Seller’s Full Name
- Seller’s Street Address
- Buyer’s Full Name
- Buyer’s Street Address
Step 5 – Purchase Information – Finalize with all the sections relative to the sale of the vehicle:
- Sale Price
- If the sale was a gift, provide the relationship of the buyer to the seller.
Step 6 – Complete with signatures on the given lines as well as the date of the signature at the bottom of the form.
Step 7 – Once you have prepared the form from the dotted line and above, complete it in the same fashion with the lower segment of the form. This will effectively furnish a second copy to give one to each involved in the sale.
