Statutes – Colorado Revised Statutes – Powers of Attorney (§ 15-14-500.3 – 15-14-509)
Definitions – “Power of Attorney” means a power to make health care decisions granted by an individual (§ 15-14-500.5(a)).
Signing Requirements – The principal party is not demanded, by law, to have their signature witnessed or the form notarized. With that being said, it is still recommended that they implement at least one (1) of these measures.
