The Illinois Power of Attorney has the ability to assign a third-party individual, known as an “agent” or “attorney-in-fact”, the authority to manage aspects of one’s life connected to medical decisions, finances, child care, etc. In order to do this, the agent must be given authorization by the principal using this form.
